WelbeHealth Expands PACE Access In Sacramento And San Bernardino Counties
Programs bring integrated care to seniors in growing and underserved communities
Menlo Park, CA /PRNewswire/ - Today, the physician-led senior health services provider WelbeHealth announced the opening of two new centers in Elk Grove (Sacramento County) and San Bernardino (San Bernardino County). The launches reflect WelbeHealth's continued commitment to expanding access across California through the Program of All-Inclusive Care for the Elderly (PACE), a national Medicare and Medicaid program.
With two of the fastest-growing senior populations in California, Sacramento and San Bernardino Counties face an urgent need for high-quality, affordable senior care. Elk Grove, the second-largest city in Sacramento County, has grown to nearly 183,000 residents, with about 24% aged 55 and older. In San Bernardino, east of Los Angeles, rates of chronic illness are among the highest in the state.
