Oregon Medical Group Selects Phytel To Support CMS Comprehensive Primary Care Initiative And Population Health Management

Dallas, TX And Eugene, OR (Marketwired) - Phytel, the leader in automated, provider-led population health improvement, announced today that Oregon Medical Group (OMG), a primary care-based multispecialty group with more than 110 providers in Eugene and Springfield, Ore., has selected Phytel's comprehensive, Web-based population health management suite as a critical component of its strategy to build new healthcare delivery models.

Oregon Medical Group will use Phytel's Outreach, Insight, and Coordinate modules to ensure that every patient receives the most appropriate intervention and follow-up care. Additionally, OMG will introduce Phytel Transition to improve care coordination for patients who are discharged from the hospital, and to reduce preventable readmissions.

Key to the medical group's decision to deploy Phytel was its role as one of a select group of physician practices nationwide to participate in a program designed to improve care and care coordination for patients. Oregon Medical Group joins a select group of practices as a participant in the CMS Centers for Medicare and Medicaid Innovation's Comprehensive Primary Care Initiative (CPCI).

CPCI practices were selected through a competitive application process based on their use of health information technology, ability to demonstrate recognition of advanced primary care delivery by accreditation bodies, service to patients covered by participating payers, participation in practice transformation and improvement activities, and diversity of geography, practice size and ownership structure.
