New CareOne Clinical Programs Enable Highest Level Of Patient Care

Fort Lee, NJ /PRNewswire/ - CareOne, a premier network of senior care centers throughout New Jersey, is at the forefront of a new delivery-of-care system enabling the highest level of patient care. Clinical specialty programs such as the STAR Program® (Survivorship Training and Rehab) for oncology, diabetes education with certification from the American Diabetes Association (ADA), and COPD (Chronic Obstructive Pulmonary Disease) management have been implemented to provide a higher standard of care and improve patient outcomes. In addition, CareOne is shaping the future of education for medical professionals by serving as a clinical site for Physician Assistant (PA) students from Seton Hall University and Practical Nursing (PN) students from Berkeley College as well as developing a collaborative Nursing Residency program with Rutgers University.

CareOne has received the first-ever STAR Program® certification delivering oncology services for in-patient, sub-acute facilities in the state of New Jersey. Through this gold-standard program, CareOne offers more comprehensive, coordinated cancer rehabilitation to enrich the quality of care and improve patient outcomes.

The American Diabetes Association (ADA) granted the first certification on the east coast to CareOne at Morris for their Diabetes Education Program. CareOne adopted these ADA standards in an effort to empower patients and caregivers to manage diabetes, improve blood glucose control, and prevent complications.

CareOne at Livingston has partnered with Saint Barnabas Medical Center to implement a seamless, continuum-of-care program for COPD patients to ensure that they receive ongoing support and education leading to long-term benefits after rehabilitation. "COPD is a disease that can be controlled," says Maria Dimi, Administrative Director of Respiratory & Neurodiagnostics Services at Saint Barnabas Medical Center. "By providing the right tools, this program empowers patients to control the disease rather than allowing the disease to control them."
