CoCENTRIX Coordinated Care Platform Unites Texas Healthcare Community

CoCENTRIX delivered the Coordinated Care Platform to the Topaz Software Authority, an organization founded by two of the largest Mental Health Mental Retardation Agencies in the state, the Mental Health Mental Retardation Authority of Harris County and Mental Health Mental Retardation of Tarrant County. As part of the project, CoCENTRIX and Topaz teamed together to develop a common taxonomy, “the Texas Best Practices.” Texas Best Practices provides organizations across the state with access to shared clinical and administrative best practice content and the ability to more readily comply with Texas-based state assessments, state reports and payer requirements.

The CoCENTRIXccp, combined with the Texas Best Practices, provides a rapid deployment methodology with the ability to leverage cloud technology to reduce costs, interoperate with other networks and structure a data repository available for data mining and outcome-based analyses. Built on Microsoft Dynamics technology, the CoCENTRIXccp enables communication, information exchange, and collaboration among providers of all disciplines, state and county oversight authorities, and participants across the entire healthcare continuum.

To date, 10 Texas Agencies have signed on as CoCENTRIX partners providing services in 53 Texas counties serving an estimated 1.5 million consumers each year. The participating agencies are: Mental Health and Mental Retardation Authority of Harris County, Mental Health Mental Retardation Authority of Tarrant County, Andrews Center Behavioral Healthcare System, Central Plains Center, Gulf Coast Center, Denton County Mental Health Mental Retardation Center, Heart of Texas Region Mental Health Mental Retardation Center, Helen Farabee Center, Pecan Valley Centers for Behavioral & Developmental Healthcare, and StarCare Specialty Health System.
